The Northern Chronic Care Coalition (NCCC) is a network of people from the Northern Health Strategy partnership with a shared interest in working together to improve chronic disease management, prevention and screening in northern Saskatchewan.
The members are northern residents that come from a variety of health care backgrounds, with a passion to improve the quality of care for people living with chronic disease.
The partners meet quarterly to:
- build an umbrella of support to launch Chronic Disease initiatives in the North;
- provide a forum for networking between partners to share knowledge, experiences and resources related to practices in Chronic Disease Management;
- bring a collective voice and advice to policy makers regarding best practices in Chronic Disease Management and Prevention in the North; and
- coordinate the efforts of each of the partners to build upon their expertise, reduce duplication and to maximize the resources in the North.
